Data Collection

It’s sad to say, but one of my favorite parts of my jobs is organizing.  Folders, printed materials, labels, dividers, hole punches, highlighters… It all makes me giddy.  There was an empty filing cabinet in the conference room; It was the HUGE kind where the drawers go lengthwise.  I asked if I could have it and my wish was granted – it was like Christmas!  I covered it in damask print contact paper and couldn’t have been more excited.  Seriously.

Anyway, I will tell you how I utilize these 4 huge drawers.  The bottom 2 are filled with books/resources that I don’t need often.  They’re mostly workbooks whose pages I would photocopy for students, but rarely do so.  The top one is filled with my personal poly-files organized by area of speech/language (red=aRtic, blue=Basic concepts, green=GRammar, yellow=Language (misc), white=pragmatics).  See what I did there?  I did those from memory and chose that arrangement because it makes sense to me.  Dork, I know.

The second drawer is filled with my students’ folders.  It’s the drawer that is closest to eye/hand level and makes reading the names on the folders easy.  Each student has a folder.  It is a beautiful, sturdy folder with a divider in it, and a clasp at the top of each.


When you open the folder, the left side has the data collection sheet.  Each data sheet has a chart that I made in Word.  The top row has each of the student’s goals (in my own, shortened words).  The first column on the left has pre-filled spaces for date, minutes, group size, and group type (all mandated requirements for Medicaid purposes).  Then there is also a “comments” column on the far right.  Each day takes up one row.  I can usually get about 4 sessions worth of data on one sheet, more for some kids.  Download an editable version of the data sheet here.

I personally need all of a student’s goals written out so I can see them while I’m working.  Yes, I tend to know their goals, but this type of chart makes it easy to see what you’ve done recently and what you may have been neglecting.  Also, let’s say you are working on artic, but the student uses a pronoun correctly (also one of his/her goals).  You can easily put a “+” in that column.  That way, it doesn’t go unnoticed and you don’t waste time writing what it was that the student did.  If the student is absent that day, or I’m in a meeting, I just write a zero on the minutes line and write the “excuse” in the first box.  It takes 2 seconds and there’s no question of why therapy wasn’t held.

With the folder open like this, the page on the right is my attendance calendar.  Each day, Monday through Friday, has a space for a code.  The codes are listed at the top of the page, for easy reference for yourself or anyone else who may need it.  They include: “W” for weather closure, “G” for group, “SA” for student absent, “TU” for therapist unavailable, and more.  It consists of 3 pages: Oct-Dec, Jan-Mar, Apr-June.  (I didn’t start using that type of attendance until October, that’s why there is no September.)  Here is an updated copy for you to try.

When the middle page/divider is turned, you’ll find the student’s IEP on the left side.  I only print the pages that are relevant to me – usually the goals and the service hours.  I print them double sided; open to the top (as opposed to the side) so that when you lift it and it is in the clasp, you can still read it without turning the whole folder.  Or, you could do one sided printing if your printer doesn’t allow for this.

The last side is the right side, where I keep extra data sheets.  That way, when one fills up, I have plenty right there.  At the beginning of the year or an IEP period, I usually print about 20 sheets and use my handy two hole punch.  I put the new one right on top of the previous one and they all end up in chronological order.  When a calendar page fills up, I also put that on top of the used data sheets.  At the end of the year, I use one of those big paper clips to keep it all together and put it all in the student’s working file.  (Those are held in another drawer that is opened far less often.)

Some of you may remember my post about students graphing their own accuracy.  I just keep this piece of paper loose between the IEP and the extra data sheets tabs.  That area isn’t opened all that often and it’s easy to pull out the accuracy chart so the student can color it.  I also stick worksheets in there too, if applicable.

During the therapy session, I take out each student’s folder.  I write the applicable code in the attendance calendar, then fold the whole folder over so that only the data sheet is visible.  I then stack them on top of each other in the order the students are sitting.

For about a year, I’ve used Super Duper’s Data Tracker app.  In the app, I do my + and – tracking.  Then, at the end of the session, I transfer the percentage to the data sheets.  However, before I used the app, I did my + and – writing right in the folders.  The app definitely makes it easier, but it’s not essential to this system.

A variation of this system is the one I used in my first clinical placement in grad school.  I have been using it from my first day as a CF and I truly love it.  It’s easy to refer to and very visual.  You know at a glance what you’ve targeted recently and what you haven’t. If you, or your district, can’t afford the folders I use, I know there are similar, cheaper, manila versions of them.

If you don’t shift to the whole system. I hope you can at least use one element of it to make your life a little easier!

      Thanks for sharing! I have a question, I love the separate folder idea, however do you pull out every childs folder for each sessions? I feel like having 4-5 files on my table would be difficult to manage. About the super duper app, I am going to try using it again looks like they have updated it since I last used it. If I load all the info in my phone and then open the app on my ipad will it transfer the info? Thanks again, you look as organized as I am! Super obsessed, which is why my current data tracking is driving me crazy! I want something easy and fast.       I really like how you've done your folders! I personally use a binder with a goal sheet and data log for each student. My previous districts required use to do medicaid billing online, so we had to retype everything anyway, and my current district is not medicaid billing this year. I really like the ease of it so I can track data quickly and concisely each session…but I might incorporate some of your ways to make it even more efficient.

      Yes, I do pull out each student's folder every time. I usually have no more than 4 students per session, so as they are sitting down I grab their folders from the drawer. After I write on their attendance calendar, I fold the folder in half so that only the data sheet it available (facing up). I stack them on top of each other, in the order the students are sitting. Then, after each student's turn, I put his/her folder to the bottom of the pile. So essentially, they really only take up as much space as one folder does because I stack them in a pile. It's easy once you get used to it. The Super Duper app is WONDERFUL! An update recently changed the app so that when you do not work on a goal, it does not show up as a “0” – both in the listed form and the graph. I don't use the graphs as my only form of data, so it's a good supplement. And it calculates percentages quickly!

      We have recently had to do our Medicaid billing online also. We actually have to do it for every student, Medicaid or not! Yes, I hate writing things twice, but I just love my folders and the visual ease of them too much to abandon them!

      Hi! We are also implementing a student data folder in our speech room. We actually have the students keep track of their own data for more ownership and buy in. We then chart their progress at the end of each quarter. It has worked wonderfully! Thanks for sharing your forms 🙂

    I was in a meeting the other day where a student ( I was in a meeting the other day where a student (whom I didn’t know) was approved for an iPad with Proloquo. It almost certainly wouldn’t arrive until the fall. Two well-meaning professionals in that meeting suggested “doing PECS” in the meantime. 

The parent was willing to purchase “whatever” until the device could arrive. I’d hated the idea of her buying an expensive, extensive PECS book for two months of use. I’d also hated the idea that it seemed no one knew PECS is actually a very specific set of steps for learning/teaching in which one should be trained before implementing. *It’s not just laminated pictures of items the child likes placed in a binder. It’s also not a magic 💊 when handed to a student.

Instead I suggested printing the Home Screen of Proloquo and beginning to teach that to this student. 

Paper-based options are completely valid and feasible. I’d say, even essential to introduce in case a device is dead or breaks. 

The ease with which this parent could give her child a “head start” on AAC via printing a screenshot is far and away more than what buying a whole PECS book would entail.

We as SLPs should be doing this. We should have these on hand. While waiting for a device and also while using one. And tell parents. Maybe they go on vacation and forget the device/printed piece. Show them how easy it is to google and print themselves! 🙌🏻

And please, can we stop referring to all collections of picture cards in a binder “PECS”…? 🥴🫠
    (I have a funny feeling by “better” you just m (I have a funny feeling by “better” you just mean *cheaper*… 🙄)

    When did tangible, cheap, plastic trinkets being s When did tangible, cheap, plastic trinkets being sent home for every holiday, from every kid in the class AND the teachers, become a thing we do?

It’s no secret that I hate all of the $ store junk my kids come home with. My “hack” is to dump it all in a bag and use it for the non-edible, allergy friendly candy in my Halloween bowl. Some kid is gonna get a 💜 pop-it that says “Happy Valentine’s Day, love Brayden” when they trick or treat at my house but 🙃 oh well!

Instead my speech kids go home with:
✅the “end of the year memories” book, tailored to speech therapy, that we completed
✅a simple one-page homework sheet for their sound that their parent will actually be able to use and not be overwhelmed by enough to ignore it all summer
✅a wave, high-five, or a hug
AND THAT’s IT!

Especially since I’m confident the majority of those pages weren’t even looked at. 

Until I made a one page instruction sheet with simple, parent-friendly instructions for how to accurately produce sounds… parents were very appreciative! 

If you’ve got students/caregivers requesting work over the summer to prevent loss of articulation skills, these one page, black ink only, no prep sheets have:
✅a mouth model visual
✅concise written instructions for production of 12 phonemes
✅words in initial, medial, and final with visuals
✅sound-loaded phrases
✅sound-loaded sentences

    Before my oldest started school, I swore I wasn’ Before my oldest started school, I swore I wasn’t going to do this trend. I figured teachers thought it was a pain and I hate Dr Seuss books.

Then my cousins, both of whom are HS teachers, told me how much they love writing in and reading their students’ books and that I just HAD to do it.

Once I found other book options, I was into it! 😍 I have 3 kids and they all have different books.

I’m Denise, a pediatric SLP, wife, mom of 3 humans (9, 6, & 3)  and 1 black lab. I live on Long Island and am currently working as a preschool/EI transition evaluator. 

My favorite communication skills to target are early language, articulation, and core vocabulary! 
I love cheese, margaritas, photography, and trash TV (Bravo).
I hate feet, olives, and weather below 70 degrees. (I’m chronically cold.)

    I was evaluating a 2 year old. He said “no” to I was evaluating a 2 year old.
He said “no” to *every* prompt in the receptive section. 
He started banging the spoons together.
I flipped everything upside down and made a drum set. 
We played the drums together.
It elicited language. Unique 3 word phrases!

This is play. This was fun.

He continued to tell me “no” but at least he wasn’t miserable the entire time 🙃

    When did tangible, cheap, plastic trinkets being s When did tangible, cheap, plastic trinkets being sent home for every holiday, from every kid in the class AND the teachers, become a thing we do?

It’s no secret that I hate all of the $ store junk my kids come home with. My “hack” is to dump it all in a bag and use it for the non-edible, allergy friendly candy in my Halloween bowl. Some kid is gonna get a 💜 pop-it that says “Happy Valentine’s Day, love Brayden” when they trick or treat at my house but 🙃 oh well!

Instead my speech kids go home with:
✅the “end of the year memories” book, tailored to speech therapy, that we completed
✅a simple one-page homework sheet for their sound that their parent will actually be able to use and not be overwhelmed by enough to ignore it all summer
✅a wave, high-five, or a hug
AND THAT’s IT!

    The number of hours I wasted standing at the copie The number of hours I wasted standing at the copier making giant summer speech homework packets in my first couple years working makes me 😵‍💫
Especially since I’m confident the majority of those pages weren’t even looked at. 

Until I made a one page instruction sheet with simple, parent-friendly instructions for how to accurately produce sounds… parents were very appreciative! 

If you’ve got students/caregivers requesting work over the summer to prevent loss of articulation skills, these one page, black ink only, no prep sheets have:
✅a mouth model visual
✅concise written instructions for production of 12 phonemes
✅words in initial, medial, and final with visuals
✅sound-loaded phrases
✅sound-loaded sentences

    Before my oldest started school, I swore I wasn’ Before my oldest started school, I swore I wasn’t going to do this trend. I figured teachers thought it was a pain and I hate Dr Seuss books.

Then my cousins, both of whom are HS teachers, told me how much they love writing in and reading their students’ books and that I just HAD to do it.

Once I found other book options, I was into it! 😍 I have 3 kids and they all have different books.

    There’s lots of new faces around here, so I thou There’s lots of new faces around here, so I thought I’d show you mine again and introduce myself!

I’m Denise, a pediatric SLP, wife, mom of 3 humans (9, 6, & 3)  and 1 black lab. I live on Long Island and am currently working as a preschool/EI transition evaluator. 

My favorite communication skills to target are early language, articulation, and core vocabulary! 
I love cheese, margaritas, photography, and trash TV (Bravo).
I hate feet, olives, and weather below 70 degrees. (I’m chronically cold.)

What are you shopping for during the sale?
    I was evaluating a 2 year old. He said “no” to I was evaluating a 2 year old.
He said “no” to *every* prompt in the receptive section. 
He started banging the spoons together.
I flipped everything upside down and made a drum set. 
We played the drums together.
It elicited language. Unique 3 word phrases!

This is play. This was fun.

He continued to tell me “no” but at least he wasn’t miserable the entire time 🙃
